Sneak peek at early course of bladder infection caused by widespread, understudied parasite

ScienceDaily (Nov. 29, 2012) ? Using standard tools of the molecular-biology trade and a new, much-improved animal model of a prevalent but poorly understood tropical parasitic disease called urogenital schistosomiasis, Stanford University School of Medicine researchers were able to obtain "snapshots" of shifting gene activity levels during the early, acute phase of what for most becomes a chronic bladder infection.

The findings, described in a study to be published online Nov. 29 in PLoS Neglected Tropical Diseases, could lead to new diagnostic and therapeutic approaches to urogenital schistosomiasis, a chronic disease that infects about 112 million people, almost entirely in developing countries and particularly in Africa, said Michael Hsieh, MD, PhD, assistant professor of urology and the study's senior author.

"Schistosomiasis is a huge global health problem and one of the most neglected of the neglected tropical diseases," said Hsieh. "Some 150,000 people die each year from urogenital-schistosomiasis-induced kidney failure. And this doesn't include deaths from bladder cancer or as a result of increased susceptibility to other infections, such as HIV, attributable to schistosomiasis-generated genital tract damage." Estimates that take into account chronic schistosomiasis' overall impact on quality of life place it on a par with other world-class health scourges such as malaria and tuberculosis.

Schistosomiasis is caused by a parasitic worm of the genus Schistosoma. Different Schistosoma species infect different organs, said Hsieh, whose research focuses on infection-induced bladder inflammation. S. haematobium, which is responsible for infection of the urogenital tract, is acquired by exposure to contaminated water. "The parasite larvae can smell fatty acids in our skin as well as detect motion. They're like smart bombs, equipped to find us."

After invading the skin and a pass through the circulatory system, the larvae mature to adulthood, lodge in blood vessels of the bladder wall and other pelvic organs and begin to lay eggs. A single worm can extend to several millimeters, lay hundreds of sand-grain-sized eggs every day and live for years to decades, Hsieh said.

In a study published recently in the British Medical Journal, John Ioannidis, MD, DSc, a Stanford professor of medicine, deplored the dearth of treatments for neglected tropical diseases including schistosomiasis. There are no vaccines for preventing it, and only a single drug, praziquantel, is approved by the World Health Organization for treating it. While cheap and relatively effective, it doesn't cure all infections. Plus it tastes terrible, which causes a surprisingly large number of people to stop taking it. Experts believe it's just a matter of time before the parasite develops resistance to praziquantel.

Despite a pressing need to come up with good anti-S. haematobium agents, almost all the existing medical literature is on other Schistosoma species. That's because until recently there were no practical animal models for S. haematobium infection. "We know next to nothing about the earliest molecular events in the bladder after eggs enter the tissue," said Hsieh. "When you try to naturally infect mice by simply exposing them to the parasite, the mice develop an infection in their liver or intestine instead of their bladder wall."

In March of this year, Hsieh and his colleagues reported, in the journal PLoS Pathogens, that they had solved this problem by injecting S. haematobium eggs directly into the bladder wall of ordinary laboratory mice. For the new PLoS-NTD study, Hsieh and his colleagues used this mouse model to synchronize the initiation of bladder infection by the parasite, thereby synchronizing the timing of post-infection changes that take place in bladder tissue so the researchers could better observe the parasitic infection's early trajectory. They injected about 3,000 parasite eggs into each of a few dozen lab mice and used microarray technology -- a method of globally measuring the activity of virtually every gene in a tissue -- to monitor thousands of genes' activity levels in bladder-wall tissue over the ensuing weeks.

The investigators set their microarray probe's sensitivity to flag any gene whose activation level either increased or decreased by a factor of two or more. They monitored thousands of genes whose activity levels were either amped up or tamped down at one, three and five weeks post-infection. These changes peaked at merely three weeks post-infection. "This is an important result," said Hsieh. "It shows that continuous waves of egg deposition, not just the initial schistosome infection, are what's generating chronic disease. So if you can kill the worms flat-out, you should be able to halt the cycle."

It would have been impossible to see this simply by observing patients, said Hsieh. "With the natural human infection, you never know when any given person is initially infected -- unless it's that unfortunate American tourist that got infected while swimming in Lake Victoria on a weeklong Africa visit," he said.

The team also noted that these gene-activity changes fell into clusters. "Very soon after infection, we saw changes in activation levels in a cluster of cancer-associated genes. Likewise with another cluster of genes involved in fibrosis, or scarring, in the bladder. This confirms that fibrosis kicks in very quickly," said Hsieh -- a not unexpected result, as parasite eggs' burrowing causes significant bladder-wall damage.

On the other hand, within the first week there was a general suppression of genes associated with maintaining the integrity of the bladder wall. Inadequate production of this group of molecules would be expected to result in a leaky bladder wall, said Hsieh, and it probably did. "This happened at a time when the mice were shedding eggs in their urine. So it probably accounts for not only a lot of urinary symptoms but a significant aspect of the disease's transmission -- the expulsion of eggs into the urine, which eventually contaminates another body of water," he said.

Finally, as has been observed in infections produced by other Schistosomal species, the Stanford team noted signs of a shift in character of the immune response toward a state that it is oriented to tissue repair and to isolating pathogens -- effectively attempting to enclose them in cement-like jails called granulomas -- in order to prevent further tissue damage. (This mode of immune response may be best-suited to combating relatively large pathogens that would be likely to withstand an attempt by the immune system to kill them outright in a frenzy of inflammatory fury.)

Hsieh's group corroborated their microarray results at both intracellular and macroscopic levels with ultrasound, microscopic and molecular assessments. For instance, discernible evidence of scarring in bladder-wall tissue coincided with microarray readings indicating fibrosis-gene activation.

"Schistosomes have evolved with mammals for millions of years," said Hsieh. "Our shared evolutionary history makes our findings in mice likely to be relevant to humans. These observations may also have implications for fibrotic disease in general. Understanding fibrosis is very important, because it is involved in cardiovascular disease, cancer and other diseases accounting for up to half of all deaths in the developed world."

The S. haematobium mouse model may also pinpoint biomarkers that enhance the detection and monitoring of incipient bladder cancer, a poorly understood disease.

  2. Aerobics Classes ? Usually high energy classes that help you move and groove to burn calories in a choreographed set of movements and exercises. This is great for burning fat and working the heart but won?t do a lot for muscle strength or tone.

  5. Circuit Training ? This is like a body sculpting class but usually without any aerobic segment. Instead you keep the heart rate up by moving through weight exercises (typically using dumbbells) quickly and then repeating the circuit. It burns calories and is nice for muscle endurance and tone.
  6. Dance Fitness ? This can be a lot of types such as jazz, cardio funk, or even Zumba. It mixes aerobics and dance moves into a fast paced routine that burns a lot of calories. Generally this is a high energy, fast paced aerobics class, but don?t expect to learn a lot of actual dance moves; you are there to sweat.
  7. Cycling Class ? These are also known as spin classes. If you like riding a bike these can be a great way to get some serious cardio work in. Classes vary from steady to variable speeds. Most come with high energy and loud music. The bikes have come a long way and are much more comfortable and sturdy. The upper body doesn?t get much work but the legs will feel the burn and get toned up while you burn calories.
  8. Kickboxing ? This is another high energy cardio classes that exchanges punching and kicking for other choreographed movements. You aren?t learning how to fight, but more training like a boxer would (which is a great workout) by getting the heart rate up for short bursts and then keeping it at a moderate level in between.
  9. Yoga ? This is a great class for any level of fitness because it works on flexibility and toning. You will sweat but not burn nearly as many calories compared to other classes. However many people reduce a lot more stress and loosen up more in yoga than anywhere else.
  10. Power Yoga ? A more intense yoga form that works on strength and flexibility more than regular yoga. You will also get a bit more of an aerobic affect.

(AP) ? Right from the start, the NFL said Gregg Williams was in charge of a pay-for-pain bounty system with the New Orleans Saints.

Now the former defensive coordinator ? who told the league about others' involvement ? is slated to be cross-examined by lawyers for players appealing their suspensions in the case.

Friday's session is part of the latest round of player appeals overseen by former NFL Commissioner Paul Tagliabue. Former Saints assistant coach Mike Cerullo faced questions Thursday, when lawyers for the league and for players spent more than nine hours in a Washington office building.

Saints linebacker Jonathan Vilma and defensive end Will Smith have said they'd be present Friday to hear what Williams has to say.

Biden hits up Costco: 'I?m looking for pies'

Vice President Joe Biden talks with Costco employees in the bakery section of the store while shopping at Costco??Best office "Secret Santa" ever? Vice President Joe Biden made a surprise shopping blitz through the brand-new Washington, D.C., Costco store on Thursday, emerging with a 32-inch Panasonic flat-screen TV, some kids' books, a big apple pie and Duraflame logs. He then brought his loot back to the White House compound.

"I'm looking for pies," Biden told Costco employee Ivey Stewart, his native guide through the wilderness of the big-box store's opening-day frenzy.

Biden, who was welcomed by Costco CEO Craig Jelinek and co-founder Jim Sinegal, flashed his membership card (yes, the vice president has his own membership card).?"In all honesty, I didn't have my own card. Jill wouldn't let me have one," he said. "I went to get my wife's card, and she said, 'No, No, get your own card.'"

The vice president stopped frequently to hug fellow shoppers and pose for photos, according to the print pool report from Lesley Clark of McClatchy Newspapers.

Biden enjoyed a couple of samples (one from the bakery section, where a crowd welcomed him with applause) and spent what Clark described as "considerable time" eyeing watches, including a $1,200 model. He put in a call to daughter Ashley to, he said, "get some guidance."

Shoppers snapped photos of the vice president as he took a spin through the store, including the frozen food section. However, he declined entreaties to check out the tires. "Hey man, I don't need tires," he told the Costco employees in that department. "I don't drive anymore." Some shoppers from Biden's home state of Delaware got especially big hugs and handshakes.

(On Twitter, the jokes came fast and furious: Was the bright-grinned Biden buying an industrial-size pack of Crest Whitestrips? Was he treating Congressman Paul Ryan to the $1.50 hot-dog-and-a-soda meal while Mitt Romney prepared for lunch with President Barack Obama at the White House?) He told reporters the kids' books were bound for a Delaware charity started by his wife, Jill Biden. His criteria? "Things I know my grandkids like."

Before leaving, Biden placed a call on the cellphone of Stewart, his store guide. Clark reported that his comments could not be heard, but Stewart burst into tears and got her own Biden hug.

Turning to politics, Biden urged Congress to extend Bush-era tax cuts that chiefly benefit the middle class. "Consumer confidence is growing," he said. "The last thing we need to do is dash that.

"It's important Congress acts now, right now," he added.

Israel's ex-FM Livni announces return to politics

JERUSALEM (AP) ? Israel's former Foreign Minister Tzipi Livni has announced she is returning to politics and has told supporters she is forming a new party to run in January parliamentary elections.

Livni told a news conference in Tel Aviv on Tuesday that her new party, called "The Movement," would aggressively pursue peace with the Palestinians.

She harshly criticized Prime Minister Benjamin Netanyahu's hardline government for the past four years of deadlock in peace efforts.

She says she "decided to give an answer to people who don't have anyone to vote for."

Livni's return adds another name to what already is a fractured centrist opposition.

She served as foreign minister and chief peace negotiator from 2006 to 2009. She left politics this year after she was ousted as leader of the Kadima Party.

Bucking the norm, a Texas entrepreneur went out of his way looking for rejection; but lo and behold, the 31-year-old was given a sweet surprise by a Krispy Kreme employee who went above and beyond to please a customer.

On his video blog that details the 100-day attempt, Jia explains:

I am going through 100 days of Rejection Therapy, aiming to have one rejection per day by making crazy requests. My goal is to desensitize myself from the pain of rejection and overcome my fear. I?m also wearing a hidden camera (a camouflaged iPhone) to have some fun with this video blog.

On day three, however, Jia encountered a serious hurdle: a Krispy Kreme employee named Jackie Braun who simply refused to rebuff his outlandish request.

As this YouTube video shows, Jia went to his local Krispy Kreme shop and asked for donuts in the shape of the Olympic rings. He needed them, he said, in 15 minutes.

He clearly expected a resounding "no" and was thus utterly amazed when Braun, a shift leader at the donut shop, delivered the tasty goods.

Within minutes of his request, Braun emerged with an imperfect, yet incredibly impressive, glazed Olympic ring arrangement:

She is "an individual who encapsulates all that is good about American customer service," declared the Daily Mail, adding that Braun didn't even charge Jia for the extra work.

"It?s only my third day and I have already failed, but I did so with such amazement and happiness. I am officially a fan of Jackie at Krispy Kreme," Jia wrote on his blog.

The donut company has since lauded Braun and her can-do attitude, posting this message from their official Twitter account on Nov. 24:

Braun's employer is not the only one who has recognized her exemplary service.

After his encounter with her, Jia started this Facebook group called "Give Jackie at Krispy Kreme a Raise." More than 3,000 people have since "liked" the page.

"She is a wonderful person. I learned a lot from her," wrote one Facebook user.

"I hope she knows how much she has done by being a nice person," said another.

Though Jia was "unsuccessful" in his Krispy Kreme rejection attempt, he has met with plenty of resistance otherwise. As his blog shows, he was denied the chance to make an announcement over Costco's intercom. In addition, he was told he could not send Santa a package through Fedex, and he was not loaned $100 from a stranger.

Incredibly, however, Jia has been allowed to play soccer in a stranger's backyard and was indulged by Cheesecake Factory staff who sang him "Happy Birthday" when it wasn't his birthday.

Flight conducts 'go around' after another plane blocks runway

A Delta flight operated by Pinnacle Airlines was set to land in Cincinnati on Saturday when it pulled up because of another plane that had not yet cleared the runway.

Delta Connection Flight 3972 from St. Louis had been cleared for landing Saturday evening at Cincinnati-Northern Kentucky International Airport, said Pinnacle Airlines spokesperson Joe Williams.

Air traffic control directed pilots of the CRJ-200?jet?to "go around" and return for a landing, Williams said. The plane landed?safety?and without incident at 7:32 p.m. ET.

"It was a routine missed approach," said FAA spokesperson Kathleen Bergen. "It was not an incident. It was a safe, routine procedure."

Passenger Patrick Steitz, of Laurel, Md.,?was among the 49 passengers on?board and?told The Associated Press?the pilot explained to them what happened over the loudspeaker. Passengers?were relieved the plane didn't attempt to land, he said.

Google Breaks All Android App Reviews, Threatens Android Fans' Safety

"A Google User" is now the number one Android game and app reviewer on Google Play, Android's version of Apple's App Store. That's because every single one of the millions of existing reviews, possibly including yours, has had its author replaced with this nameless, faceless person.

Screenshots taken by Jeremiah Rice of the Android Police blog show this prolific (but completely generic) author has taken over the Google Play store. Meanwhile, if you visit the store on your Android smartphone or tablet you won't see a name attached to most reviews at all; just the review's title, and the device that the game or app was run on.

Believe it or not, this is all intentional. It's the start of a new Google policy ... one which may threaten some Android fans' safety or privacy.

?Google+, whether you like it or not

Google now requires you to have a Google+ (pronounced "Google Plus") account in order to leave reviews on Google Play, the Chrome Web Store, and Google Maps. No reason for the switchover is given in the pop-up which explains this; you simply click "Continue" if you want your reviews tied to your Google+ account, and if you don't want them linked you don't write them at all. If you don't have a Google+ account, you have to sign up for one before you can write a review.

?Why Google is Plus-ifying everything

Google's success as a company is determined by how many ads it sells. Google's share of the ad market is being eaten into by Facebook, which has essentially "walled off" a huge part of people's day-to-day lives in a place Google can't index or sell any ads on. For better or for worse, Google's execs feel that what they need to do to compete is copy Facebook, in the form of Google+.

Why? Because if everyone is "Plusing" things instead of "Liking" them, and if everything people do shows up on Google+ instead of Facebook, then now Google (instead of Facebook) knows what you're doing online and where you're doing it -- and that gives it a much better position from which to display and sell ads.

?Why this is a problem for many

Besides the obvious privacy concerns (although Google offers limited tools to manage how much it tracks you), Google+'s "real names" policy is dangerous to anyone whose safety is jeopardized by attaching their given name to their online activities. This includes women who are victims of stalking, minors who are victims of abuse, transgender persons in transition, and dissidents in repressive political or religious regimes. By requiring a Google+ account to use more and more of its services, Google is forcing these people to choose between excluding themselves and running the risk of having ?all? of their Google services terminated for a "real name" policy violation, including their personal Gmail accounts.

Google+ policy allows for pseudonymous accounts, if you're widely known by that pseudonym online. Everyone's Google+ page, however, has a button to report what anyone feels is a suspicious name, which puts marginalized persons like those listed above at the mercy of every "troll" who comes by.

That?s the hard lesson being learned Tuesday by China?s ruling Communist Party newspaper, The People?s Daily, after it ran a version of a story by American satirical news site,?The Onion, that named North Korean supreme dictator, Kim Jong Un, as the ?Sexiest Man Alive for the year of 2012.?

The government newspaper didn?t just proclaim Kim the winner of the dubious honor. It positively reveled in it.

"With his devastatingly handsome, round face, his boyish charm, and his strong, sturdy frame, this Pyongyang-bred heartthrob is every woman's dream come true,? quoted the newspaper from The Onion. "Blessed with an air of power that masks an unmistakable cute, cuddly side, Kim made this newspaper's editorial board swoon with his impeccable fashion sense, chic short hairstyle, and, of course, that famous smile."

The Chinese paper?s three paragraph piece on its official website was followed by a 55-page photo gallery depicting Kim at his best ? riding a horse, shown on the cover of Time Magazine, inspecting fruit and of course, being met with rapturous applause by his people.

North Korea?s official state media, KCNA, has not commented on its website about either article.

This?wouldn't?be the first time that Chinese state press has fallen for The Onion's satire, and it remains unclear whether editors at the People?s Daily knowingly posted the piece.

In 2002, the Beijing Evening News published another story from the prank website that claimed the United States Congress was threatening to leave Washington, D.C., and relocate to Charlotte, N.C., or Memphis, Tenn., if ?its demands for a new, state-of-the-art facility are not met.?

In February of this year, U.S Congressman, Rep. John Fleming (R-La.), was left red-faced after he reposted an old Onion story that claimed Planned Parenthood was opening an "$8 billion abortionplex"?in Topeka, Kansas.

Meanwhile, this past September in the lead-up to the U.S. elections, Iranian state media fell for another Onion gag that said most rural white Americans "would rather vote for Iranian President Mahmoud Ahmadinejad than U.S. President Barack Obama.?

The operation led to congressional inquiries. It was exposed after two of the illegally obtained weapons were found at the scene of the 2010 fatal shooting of Border Patrol Agent Brian Terry.
